He is increasingly short of breath and is now on 100% oxygen via non-re-breather mask. An ABG reveals: pH 7.20 \/ PaCO2 68\/ PaO2 102\/ HCO3 28. A chest x-ray reveals bilateral pulmonary infiltrates. Potassium<\/p>\n\n\n\n<p><strong>Practice Question #3<\/strong>: Which of the following medications should be used with caution in a patient with a right ventricular infarction who is experiencing tachycardia and hypotension?<\/p>\n\n\n\n<p>A. Calcium channel blockers and ACE inhibitors<\/p>\n\n\n\n<p>B. Morphine and nitroglycerin<\/p>\n\n\n\n<p>C. IV fluids and aspirin<\/p>\n\n\n\n<p>D. Plavix and dobutamine<\/p>\n\n\n\n<p><strong>Practice Question #4<\/strong>: A patient from the psych unit was admitted for close monitoring after drinking 8 liters of water. You would expect which of the following:<\/p>\n\n\n\n<p>A. Hyponatremia<\/p>\n\n\n\n<p>B. Hypernatremia<\/p>\n\n\n\n<p>C. Hypokalemia<\/p>\n\n\n\n<p>D. Elevated serum osmolality<\/p>\n\n\n\n<p><strong>Practice Question #5<\/strong>: You\u2019ve been caring for a 43-year-old male s\/p colectomy for colon cancer. You notice when his family comes to visit, he stops conversing with you and doesn\u2019t engage or look at his family. Which psychological defense mechanism is he displaying?<\/p>\n\n\n\n<p>A. Withdrawal<\/p>\n\n\n\n<p>B. Repression<\/p>\n\n\n\n<p>C. Regression<\/p>\n\n\n\n<p>D. Sublimation<\/p>\n\n\n\n<p>Now let\u2019s see how you did:<\/p>\n\n\n\n<p>#1: <strong>B. Acute Respiratory Distress Syndrome. <\/strong>Criteria for ARDS include: 1) Timing &#8211; development of worsening symptoms within 1 week of insult. 2) CXR reveals bilateral pulmonary infiltrates. 3) Non-cardiogenic pulmonary edema. 4) Oxygenation &#8211; P\/F ratio &lt; 300. ARDS is further classified as mild, moderate or severe based on the P\/F ratio. To calculate the P\/F ratio, divide the PaO2 by the FiO2. In this case 102 (PaO2) \u00f7 1.0 (FiO2 &#8211; 100% O2) = 102, making it borderline severe ARDS. The patient needs to be transferred to the ICU and will likely require intubation &amp; mechanical ventilation.<\/p>\n\n\n\n<p>#2: <strong>D. Potassium. <\/strong>Patients taking angiotensin converting enzyme inhibitors may experience hyperkalemia. ACE inhibitors block angiotensin II, which indirectly blocks aldosterone. Aldosterone is responsible for excreting potassium from the kidneys. Therefore, ACE inhibitors can cause potassium retention. In addition, renal labs such as BUN and creatinine should be monitored. If the patient develops &gt; 30% increase in the creatinine, the medication should be discontinued.<\/p>\n\n\n\n<p>#3: <strong>B. Morphine and nitroglycerin. <\/strong>A patient with a right ventricular infarction presenting with tachycardia and hypotension is showing signs of decompensation and shock. Often when the right ventricle is infarcted, the wall motion is stunned causing a reduction in forward flow of blood from the right to the left side of the heart. To improve forward flow, the treatment is IV fluid to maximize preload (think Starling\u2019s Law!). Patients with right ventricular infarctions can become preload dependent. Medications that decrease preload should be avoided. Examples include morphine, nitroglycerin, beta blockers and diuretics.<\/p>\n\n\n\n<p>#4: <strong>A. Hyponatremia. <\/strong>Free water has a hypotonic tonicity. Since the tonicity or osmolality is lower than serum osmolality, it can dilute the serum sodium and drop the serum osmolality. Ingesting large amounts of free water in a short period of time can cause life threatening hyponatremia as it can lead to cerebral edema.<\/p>\n\n\n\n<p>#5: <strong>A. Withdrawal. <\/strong>Withdrawal is a psychological defense mechanism that patients use to separate themselves from others to avoid emotional responsiveness. Repression is when a patient unconsciously forgets painful thoughts. Regression is when a patient goes back to an earlier level of emotional development. Sublimation is when a patient diverts their energy derived from unacceptable impulses to a more acceptable social or moral use.<\/p>\n\n\n\n<p>Whether you got all 5 questions correct or only 1, it is okay! Each practice question you answer is an opportunity to learn and refine your understanding.
